You could also fax the form to 770-435-0402 or insert it into the shipping container with the equipment you are returning.
It is suggested that you keep a copy for your records. If there are any questions or help is needed with this form please call 770-435-3309. When calling to check the status of your equipment, having your account number handy will greatly speed up the process.
